Summary

IRB-approved consent forms (English and Spanish) for one-time community focus groups regarding community perception of environmental exposures and health risks, as well as perceptions of ways to mitigate risks at the household level. In addition, the form also asks for input on recruitment and retention strategies and protocols for in-home visits.

Note: This consent form example from 2024 included the Injury Paragraph and other sections that may not be applicable to other studies. SDSU investigators are encouraged to always create consent documents for their study from scratch using the most current consent templates located on the SDSU Human Research Protection Program webpage HERE. Consent documents created from a previously approved consent example and/or templates will likely result in stipulations to approval and increased time to final approval.
